Role of Music Therapy in Dementia

Dementia:

Dementia is a progressive neurologic disorder that changes behavior, diminishes cognition, and deteriorates memory due to a disease or injury. Some causes of dementia which may or may not be reversible are brain injury, use of certain medications, hypoglycemia, hypothyroidism, immune disorders, vitamin B12 deficiency, excess alcohol intake, and smoking. The common types of dementia are Alzheimer's disease, Lewy body dementia, and vascular dementia.

The most pronounced effects of dementia are on memory and visual-spatial. Some psychological and behavioral expressions that can manifest are aggression, agitation, depression, wandering, restlessness, and trouble eating or swallowing. During the late stages of disease, difficulty in swallowing can result in breathing food into the lungs that may lead to aspiration pneumonia.

Treatment:

The symptoms associated with behavior and psychology affect patients and their caregivers. Available pharmacologic treatments used to treat behavior have little benefit and significant risks. Due to increased risk of mortality associated with these drugs, the FDA has issued warnings against their use especially in elderly patients. The Dementia Action Alliance encourages integrated approach to focus on a person's behavioral and psychological expressions rather than following general practices.

In a holistic approach patient-specific behavior is identified and modified to eliminate conditions which contribute to a specific behavior. A targeted approach provides patient activity program and builds skills that simplify communication and tasks. Various therapies that have been used to support a person living with dementia are music therapy, art therapy, reality orientation, aromatherapy, and cognitive behavioral therapy.

Music therapy:

It is a health profession in which music is used as a therapy to improve mental, physical, and social wellbeing of an individual. It helps to balance spiritual and emotional needs of an individual to improve quality of life.

The qualified music therapist provides treatment based on individual patient's needs and may include playing musical instruments or video games, singing, dancing, song drawing, listening to music, and / or multisensory stimulation. It can be provided as individual or group therapy, however studies have shown more positive results with group therapy. Active therapy engages patient with direct participation while passive therapy allows patient to listen to music or engage in another activity. Active therapy has been found to be more helpful in improving physical functions of the patient like grasping an object.

Credentialing of music therapist:

Music therapy is a health profession based on evidence. The therapist must earn at least a bachelor's degree, complete 1200 hours of internship and obtain MT-BC credential issued by the Certification Board for Music Therapists to become a certified music therapist.

The therapist may hold different designs like Certified Music Therapist, Advanced Certified Music Therapist, or Registered Music Therapist. He must obtain continuing education credits and hold licenses in states that require board-certification to safeguard competent practice.

Impact of music therapy:

When we listen to music different parts of brain become stimulated. For instance, music stimulates amygdala which is involved in processing of emotions. Dancing and playing an instrument involves motor cortex which controls movement. Musical experiences excite hippocampus which is responsible for memory and spatial navigation. It increases blood flow in the brain, strengthens executive functions, supports heart, improves communications, and reduces stress.

Studies have shown the positive effect of music therapy on psychological and behavioral symptoms of dementia. A study by Ozdemir L et al., 2009 [1] improved cognition and reduced depression and anxiety with continued effect for three weeks following the completion of study in mild dementia. Another study by Li CH et al., 2015 [2] showed that cognition in music therapy group was reduced less as compared to the control group and change of abstraction domain in the music therapy group was better.

Numerous other studies have been done using different assessment instruments like Mini-Mental State Examination, Geriatric Depression Scale, Beck Anxiety Scale, Hamilton Anxiety Scale Scale, Clinical Dementia Rating, Cognitive Abilities Screening Instrument, and Neuropsychiatric Inventory. Several studies have shown promising results in the form of decreased anxiety, positive emotional states, and increased relaxation.

It has the potential effect to enhance the quality of life, improve neuropsychiatric symptoms, and reduce symptoms like cognitive decline and depression. It benefits patients who have difficulty communicating or expressing themselves in words. This, in turn, strengthens patients' abilities and transfers it to other areas of their lives.

Conclusion:

A clinician can play a role in facilitating collaboration with specialists and other healthcare professionals to implement music therapy. Researchers have demonstrated that music therapy can protect cognition, executive function, psychomotor speed, and autobiographical and episodic memories and can yield high levels of patient and caregiver satisfaction. Additional clinical trials will add evidence to support the positive effect of music therapy. Multisensory stimulation with dance, art, video game, and physical exercise seems an exciting and promoting approach.

Role Playing Text Games: MUDs and Music

Music is a large part of life, and it can enhance almost everything … so it makes sense that it can affect your role playing game experience. A solid music selection can really make your MUD experience that much more dramatic. Sure the background noise of television is alright to listen to while playing your favorite text-based game, but music enables you to feel more emotions, and extremely get more enjoyment out of it. You can act as your own DJ to figure out what songs are best for you to listen to while you role play.

Death Metal, Classical, Pop … any of these genres could have the key to enhancing your actions in the text-based world that much more. Depending on your mood, it could change day to day what you feel like listening to, and what fits with your role playing game experience. What bands and songs you listened to can even influence how you take on events in your character's life. Music could be that missing element that helps you fully immerse yourself into your character. And depending on your character and the situation, there are a range of different music styles that you can choose to listen to while playing your text game.

Imagine what fun could have been listening to Michael Jackson's "Thriller" while taking on some undead. Do not deny your other senses – your eyes may be all that you technically need to play, but your ears can offer just as much. In fact, all of the senses can enhance your role playing game experience; music is just an easy and oft-preferred method, with a multitude of studies behind it to back its use up. Music has been used for ages to strengthen moods, and help with things such as depression, fatigue, relaxation, and much more.

One of the main things a person will often remembers after playing a game is the music used to complement the gameplay. The perfect note or set of lyrics could could really strike a chord with your character's life and create a forcefully evocative experience. By listening to songs while going through personal events in-game, there is a possibility for a much stronger and more lying experience to be had. Music is a major source of connection and identity. Personal taste next, you can even still dislike a song and have it draw forth a powerufl emotional reaction such as hate, dislike, fear or disgust.

Role playing experiences (like those found in a role playing MUD) and music can really go hand in hand together to provide a much richer experience. The many ways one can personalize the two and create an even more truly unique experience is unlimited. A new song or a new change in your game could result in something completely fresh. So the next time you log on make sure not only to use your eyes, but use your ears as well.

It is a way of expressing our feelings and emotions. Music is a way to escape life, which gives us relief in pain and helps us to reduce the stress of the daily routine. It helps us to calm down, an even excites us in the moment of joy. Moreover, it enriches the mind and gives us self confidence.

Music surrounds our lives at different moments of lives, whether we hear it on the radio, on television, from our car and home stereos. Different kinds of music are appropriate for different occasions. We come across it in the mellifluous tunes of a classical concert or in the devotional strains of a bhajan, the wedding band, or the reaper in the fields breaking into song to express the joys of life. Even warbling in the bathroom gives us a happy start to the day. Music has a very powerful therapeutic effect on the human psyche. It has always been part of our association with specific emotions, and those emotions themselves have given rise to great music.

The origins of Indian music can be traced back to the chanting of the Sama Veda nearly 4,000 years ago. The primacy of the voice, and the association of musical sound with prayer, were thus established early in the history of Indian music. Today, music is available for us in different forms and the choice for music varies from person to person just as the reading choices vary from one another. There is folk music, classical music, devotional music, instrumental, jazz, rock music, pop music, hindi movie songs and many more.

In the modern world, Music has gained an honourable designation of ‘HEALING WITHOUT MEDICINE’. Doctors feel that music therapy has been helping them in treating many people with problems like dementia, dyslexia depression and trauma.” Many children with learning disability and poor co-ordination have been able to learn, and respond to set pieces of music. Many people with genetic disability have found a new light in the form of music.

Dance critic Ashish Khokar cites an experiment as proof: “Music is produced from sound, and sound affects our sense perception in many ways. Even fish in an aquarium were once made to listen to different kinds of music and it was found that their movements corresponded with the beat of the music. Mind you, fish do not hear, they only felt the vibrations of the sound through water. So you can imagine what a profound effect sound and music might have on the human mind.”

Anand Avinash, founder of the Neuro Linguistic Consciousness workshop who has researched music therapy says,”the mystics and saints from ancient to modern times have shown how music can kindle the higher centers of the mind and enhance quality of life.” Mantras, or chants used in the West, repeated monotonously, help the mind to achieve a sense of balance. A combination of the sounds in Sanskrit mantras produce certain positive vibrations and elevate the mind to a higher lever of consciousness.

We all know that meditation cleanses the system of its negative energies and vibrations. And music is a powerful aid to meditation. In many meditation workshops, music is used to make people more aware of their moods and feelings. People are made to lie down and empty their minds and then listen to the music which is systematically changed so that they can fit through different emotions and state of consciousness.

Many people also believe that any music you respond to positively will work for you, regardless of its content. Thus, even pop music might work wonders for you.

Music affects all of us in some way or the other. It also is the most common interest of many people. People who love music, listen to it while traveling, reading, meditation, walking, some even have soft music while working in their busy routine. It helps them to relax and escape from the stress of our day-to-day lives. It can transport us to another time or place and it is a great feeling of seeing or doing or experiencing something different. People have special music corner for themselves and some people give importance to listening in silence and some people love to read with light music and even some people love listening to music before sleeping. Many people love listening to music in bathroom because they feel it is one of the few rooms in the home where privacy is routinely respected. Some people also love to sing in the bathroom and are called ‘bathroom singers’. Music has now become a part of our life as it serves different purposes for each one of us.

  • It serves as an entertainment tool. For instance, in an occasion or event, music plays a vital role that makes the event to be lively for the people. Similarly, it creates cordial relationship among the people.
  • Moreover, it serves as a tool for corrective measure. Music tell the people on the habit that is uncultured so that such behavior can be for better. Furthermore, it is an agent that is used to educate people. Music can easily convey message to the friends and enemies.
  • It serves as tool for settling dispute between two or more people. It often helps to put an end to disagreements after listening to related meaningful songs. Music is played for the group to show harmony among them.
  • Music also serves as a source of income to human life. It is a profession of particular classes of people like lyricist, playback singers, music directors, musicians, musical instrument players, djs etc.
  • Lastly, music serves as a message or symbol that indicates the occurrence that is going on in a particular place or event. For instance, If bad occurrence happen in a particular place the type of music played their will show the audience or listens what happened in that event. The type of music played will justify to the listeners what actually going on there.

So today has been a big day for Black Sabbath fans! The black sabbath 11.11.11 press confrence has officially finished and here is some of Metal Hmmers play by play of the press conference. So it started off with a video montage with some old black sabbath videos then the special guest of the announcement “Henrey Rollins” came out and talked for a bit. Some quotes from his speech were, When I heard Black Sabbath, I realized my life had a soundtrack. “audiences are not stupid- they know what’s real and Black Sabbath is real.” After Rollins talked for a bit they announced they will be recording a new album staring in the beginning of 2012 with Rick Rubin producing the record. They also announced they are half way through the writing of this album. Then Sabbath same out! then Bill Ward took a moment to honour the world’s veterans. Then sabbath answers reporters questions, Why now? “It was now or never, really. We have some great music to play.” Tony answered. Ozzy then stated “it was time”.
Geezer then said, “The stuff that Tony’s been playing is brilliant”. So there will be a new 2012 black sabbath full length record. They will be headlining download festival 2012 on the Saturday and a 2012 world tour will follow!
